It is meant for those who have to take care of patients who are not able to swallow their secretions or mucous because of various respiratory and neurological conditions. Secretions left in the tube could become contaminated and a chest infection could develop. To prevent lung infection and maintain good oxygenation proper suctioning of oral and tracheal secretions is required. All tracheostomized patients require regular suctioning. The secretions should be white or clear. If they start to change colour, (e.g. yellow, brown or green) this may be a sign of infection.

What is Tracheal Suctioning with a Portable suction machine?

Tracheal Suctioning is a means of clearing the airway of secretions or mucus through the application of negative pressure via a suction catheter. For tracheal suctioning a portable suction machine should be kept on bedside and within easy reach.

When should you do suction of your patient with a Portable suction machine?

1. Audible or visual signs of secretions in the tube.
2. Suspicion of a blocked or partially blocked tube.
3. Inability by the patient to clear the tube by coughing out the secretions.
4. Vomiting.
5. Desaturation on pulse oximetry.
6. Any time the patient feels or hears mucous rattling in the tube or airway
7. In the morning when the patient first wakes up
8. When there is an increased respiratory rate (working hard to breathe)
9. Before meals
10. Before going outdoors
11. Before going to sleep

What are the Safety considerations while doing suctioning with a Portable suction machine?

  • Tracheal damage may be caused due to suctioning if not done properly. 
  • This can be minimised by using the appropriately sized suction catheter, appropriate suction pressures and for optimum duration. 
  • The pressure setting for tracheal suctioning is 80-120mmHg (10-16kpa). To avoid tracheal damage the suction pressure setting should not exceed 120mmHg/16kpa.
  • Each episode of suctioning (including passing the catheter and suctioning the tracheostomy tube) is completed within 5-10 seconds. This is repeated 2-3 times until trachea is cleared of secretions

How to Suction using a portable suction machine?

  1. Gather the supplies. 
  2. Wash your hands: Wash your hands with soap and water and dry them with a clean towel.
  3. Wear disposable clean gloves
  4. Check portable suction equipment and make sure all tubing connections and collection jar have a tight seal.
  5. Place the portable suction machine on a flat, dry and safe surface. Plug the suction machine to an electrical outlet and turn it on.
  6. Set the suction pressure
  7. Check pressure by turning the machine on and covering the open end of the suction line with one hand.
  8. Pick up clean suction catheter.
  9. Put on gloves.
  10. Connect the tubing to the suction catheter.   
  11. Place the catheter into the trach tube and be careful not to let it touch anything before entering the opening. 
  12. Slowly move catheter down into the trachea until resistance is felt. Do not force the catheter down.
  13. Cover the suction port with your thumb and slowly pull the suction catheter up and out. 
  14. Do not leave the suction catheter in the trachea for more than 10 seconds.
  15. Repeat 2-3 times until the secretions are gone
  16. Disconnect the catheter from the suction tubing and set it aside for washing.
  17. Rinse the suction tubing with normal saline until it is cleared of mucous.
  18. Shut off suction unit.
  19. Remove your gloves and wash your hands
